Request a Sales and Use Tax Report Form

You may request that a packet of Sales and Use Tax Report Forms be mailed to you.  To receive the blank forms, simply complete the information requested below and submit your request.  The forms will be mailed to you within two days of your request.  If you have any questions concerning the new Sales and Use Tax Report Form, please send us an e-mail.
